Gender not specified is something I've actually seen come up.

Oh, I don't mind at all, but do you really want to know?

Seriously, if I recall correctly, and it has been awhile, I saw it on one of the example reports used when I was going through a certain type of training in the military.

I can only imagine the "Don't ask, don't tell" jokes going through everyone's mind as they read that portion of my post.
