The Prime Material Plane is what you and I would consider to be the real world...

As for the other planes... Well, there's a whole host of them. You've got the Inner Planes, the Outer Planes, etc...

IIRC, the Inner Planes includes the elemental planes, and I think the energy ones as well. I think the Plane of Shadow, the Astral Plane, and the Ethereal Plane are all Inner.

The Outer Planes are, to put it most simply, the Heavens and Hells and places in between. You've got the Seven Heavens, Bytopia, Elysium, the Nine Hells, the Abyss, the Plane of Concordant Opposition...

Look at it this way: take an element or philosophical concept, and there's a whole plane of it.

I'd recommend laying hands on a copy of Manual of the Planes (either edition will do) for more info on the planes in general.

The prime never really got detailed but all we know about it is that it contains the crystal spheres of Earth, Greyhawk, Faerun, Krynn, and many other prime worlds.

The Astral/Ethereal are not inner planes they are seperate planes by themselves. The Astral is the plane of thought that is the go between the Prime and the outer planes.

Ethereal is the go between of the prime and the inner planes. (Note: 3E changed this, thus making the Ethereal basically worthless). The Shadow plane was a demiplane within the Ethereal. The Ethereal also contained Ravenloft, the demiplane of time, and many others.

For some of us, books are as important as almost anything else on earth. What a miracle it is that out of these small, flat, rigid squares of paper unfolds world after world, worlds that sing to you, comfort and quiet and excite you... Books are full of the things that you don't get in real life - wonderful, lyrical language, for instance, right off the bat. - Anne Lamott, Bird by Bird

It should be noted that the above answers depend upon the cosmology adopted. The 3e version of the Realms presents a cosmology that differs from the 2e one (and so it would appear that Oerth is now in an alternative Prime rather than the same one as Toril).

An extract from the famous "What is Planescape" discussion -
quote:
Lets start with the basics.

You know about the Outer planes, do ya prime? Well, that's a start. There are seventeen outer planes on the Great Ring:

Mechanus (first, because it's the Sage's favorite )
Limbo
Panedmonium
The Abyss
Carceri
The Grey Waste
Gehenna
Baator (pronounced Bay-ator)
Acheron
Arcadia
Mount Celestia
Bytopia
Elysium
The Beastlands
Arborea
Ysgard
and the Outlands in the middle.

In time, you might hear about places called the Astral and the Ethereal, as well as the inner planes, but enough for now.

Each and every one of those planes is infinitely big. Fit that into your brain-box for a while. Got it? Well, then I'll tell you that most of them have layers, each of which is infinitely big.

Kinda makes travel difficult, when the distance is infinite, right? Wrong. There's all sorts of ways...paths (like the Styx or Yggdrasil), gates, conduits, vortexes...and portals. And Sigil is the city of portals, the city of doors. The Cage.

It sits above the infinite spire, in the dead middle of the Outlands, completely inaccessible, except through it's portals. But it's portals go everywhere.

Further -
quote:
The planes - based on concepts like law, chaos, good, evil, earth, fire, water, air, thought or dream. The planes are usually inhabitable by mortals, who live precarious existences alongside the souls of the dead and the servants of the Powers.

If you wish to know more, just ask, and I'll provide you with links to some very informative websites.
