In the 1st edition Fiend Folio entry for death slaadi, it's said that "only four of these fearsome monsters are known to exist."
This was back in the day when Balor was the name of an individual Type VI demon. In later editions, especially considering the black and white slaadi in the ELH, undoubtedly there are more than four death slaadi.
However, for the sake of curiosity...
Does anyone recall seeing any of the four original death slaadi named in 1e sources? Have any slaadi (of any variety) been named in 2e/3e/4e sources?

* Bazim-Gorag got turned into a slaad lord. * The Guardian of the Stone was an especially powerful death slaad who watches over the Spawning Stone in Limbo. * Nurn was a death slaad servant of Hethradiah, one of Demogorgon's heads. * Zgotar was a death slaad living in the Hills of Maerth but also turned up on Oerth. * Azriim, Dolgan, Serrin and Eleura were green slaadi working for the Sojourner who turned Azriim and Dolgan into grey and then later death slaadi. * Xanxost was a blue slaad who 'authored' a couple of chapters in some Planescape sourcebooks.

A nice start, thank you! I even managed to find sources for most of them. In case anyone else is looking for them:
Bazim-Gorag is described in Champions of Ruin, pages 152-155... bonus: he's served by another death slaad, Jendar Tholm (though this is undoubtedly the name used by his human/oid form) Nurn is a recurring NPC in the Bastion of Broken Souls module Zgotar is mentioned in the Threshold of Evil adventure in Dungeon #10 Azriim, Dolgan, Eleura, and Serrin are characters in the Erevis Cale trilogy of novels by Paul Kemp Xanxost is developed in Faces of Evil: The Fiends and The Inner Planes
I haven't found a source for the Guardian of the Stone, though.

3rd ed Manual of Planes while discussing Limbo. Can't give you a page ref right now.
* Wartle is a slaad lord named in Tales from the Outer Planes who was banished to Celestia. * Chourst is a slaad lord mentioned in Dragon #221 and On Hallowed Ground. * Sorel was Ygorl's lieutenant. * Rennbuu is another Slaad lord, also mentioned in Dragon #221. |

Ssendam, Ygorl, Chourst, and Rennbuu are described on pages 72-78 of Dragon 221, and page 52 of On Hallowed Ground. Sorel is also mentioned in the Dragon article. Ssendam and Ygorl are mentioned briefly on page 99 of the 1e Manual of the Planes Ssendam, Ygorl, and the Guardian of the Stone are mentioned on page 96 of the 3e Manual of the Planes Wartle is developed on pages 48-55 of the Tales of the Outer Planes Ygorl and his "priest" (void slaad) Skirnex are also statted on pages 156-158 of the 4e The Plane Below |

Personally I wouldn't rank Bazim-Gorag as a slaad lord, because it's debatable if he was ever an actual slaadi. He was an extremely powerful Batrachi, and while it's possible that he died and his soul became a slaadi, he might have simply adopted the title and self-identified as a slaad lord.
Either way he was CE rather than CN, and seemingly as a result of that alignment conflict alone, he was forcibly exiled by Ssendam and Ygorl. |

Lord Karsus, I think you're mistaken. Ygorl was featured in Demon Stone.
Bazim Gorag wasn't exiled from Limbo, he was summoned back to Faerūn and got trapped there. |

Dregadzel is a gray slaad which serves Bazim-Gorag in Faerun, mentioned on page 154 of Champions of Ruin. |
