One time I wondered when I heard that you will make an explosion trying to teleport with magic extradimensional containers. Are there any conditions to other dimensional spells or magical items, leading to catastrophic results?

I would say that casting any form of translocation spell (dim door, teleport, gate etc) within a wild zone might trigger a rip in the fabric of space and time (ahem) and send folks to others planes, demiplanes and worlds.
Similarly casting destruction spells at gates (err portals) may do the same (trying to disintgrate a gate would probably not be a good idea!).
Snapping a staff of the magi could trigger a planar shift in 1E and 2E (I have no idea with the new ruleset).
Oh the use of artifact powers should probably do the same - plus you have the cubic gate magic item (does that still exist?) or how about the wrong command word said with a genie/efreeti bottle dragging you to their realm rather than the other way around ?
I think lots of fun can be had with demi-planes
