Who has run this? It looks awesome, but I'm noticing that the first 100 or so pages is simply masses of background information. I like reading, but how much of this needs to be really internalized in order to run this efficiently?

Not to be a jerk, but did you read the first few pages of the introduction? You could safely skim chapter 1 on the Shining Vale, but since the party will be in Daggerford, you probably want a more thorough skim of that chapter, that's roughly page 13-51 and pages 52-84. All that said, if you're players go off narrative, reading those 70 pages could come in handy. Pages 85-201 are basically the adventure, although the magic items and monster stats are in the appendices, which tacks on an additional 90 pages or so.

I think the idea is to use Under Illefarn Anew to get more background on the characters, but to actually run the adventure, you use Under Illefarn (with sticky notes to update the stats of the enemies?)

Athreeren, Eric completely updated the module. You don't need the original Under Illefarn at all to run Under Illefarn Anew.

I know, I am currently reading it actually. It's very interesting, but like Ozreth, I would find it daunting to actually use it in game. Whereas Under Illefarn is very straightforward, but presents the opposite problem that it either requires to keep the players on rails, to improvise, or to complete it. That's why I think they go well together, with Under Illefarn on the table, and Under Illefarn Anew close at hand.
