When a DM or GM is creating a custom world. Where should they start?

I say start with deities. Any fantasy realm needs deities to grant divine spells, give plots and create adventures small and large. Everything else in D&D stems from gods and goddesses. just my opinion.

Start small. Build a small city or large town for your PCs to be from or use as their base. Start expanding out from there, as you need to.

Think of matters like trade, and politics, and geography. Why is this town where it is? What resources does it have? What resources does it need? Who does it trade with? Who are the neighbors, and how does it get along with them?

And then expand as you need to, answering these questions every time, and thinking of how it all relates to what you've already done.
